General information
Description of the project
The project involves the establishment of a department of supported living and inpatient care for the elderly and people with disabilities on the basis of the premises of the former hospital in the village of Kovalivka, Poltava urban territorial community. The project involves the creation of a department of supported living and inpatient care for the elderly and people with disabilities, which will provide appropriate living conditions, care, medical supervision, social adaptation and psychological support. The implementation of the project will contribute to reducing social isolation and improving the quality of life of vulnerable categories of the population.

Description of the purpose of the project and ways to achieve it
Creating appropriate living conditions, care, medical supervision, social adaptation and psychological support for the elderly and people with disabilities who, due to age or health, cannot independently provide for their own needs, in order to improve their quality of life, integration into the community and promote active longevity. The implementation of the project will contribute to the development of a social support system that ensures a dignified, safe and fulfilling life, increasing the availability of social services and strengthening the capacity of the community in the field of social protection.
Problems and solutions resulting from project implementation
The implementation of the project allows solving a number of key problems of the Poltava community related to the lack of conditions for long-term care and supported living for the elderly and people with disabilities. Today, the community does not have a specialized department that provides comprehensive care, rehabilitation and psychological support, and a significant part of vulnerable people is in a state of social isolation and does not receive the necessary services. An additional problem is the critical technical condition of the building of the former medical institution in the village of Kovalivka, which has a high level of physical wear, outdated engineering networks and does not meet the requirements of barrier-free and safety.
The creation of a department of supported living and inpatient care will provide access to high-quality social, medical-social and rehabilitation services, improve the living conditions of recipients, contribute to their social adaptation and reduce isolation. As a result of the major renovation and modernization, the building will be brought into line with state standards, equipped for low-mobility groups of the population, and provided with modern engineering networks. The project will strengthen the community's capacity in the field of social protection and will become the basis for the further development of the supported living model.
